Meet Report Section

This is the link Page to the online Meet report Form.   This form will only work if you have MS WORD program in your computer.   Use this link to open the online  entry form:    for Meet Report Form click here   If you do not have MS WORD send us an e-mail using the contact us button on the home page, and request a form be mailed to your club.  You will need to contact the State Rep., District Director for you district, any club officer to have a meet placed.  The name of the official placing the meet must be on the report.

Instruction on how to send this form:

1.    Save the form in your MS WORD files.

2.     Open the file and type in the information requested.

3.     Save you work.   this will give you a copy of the completed form.

4.      Send us and e-mail and send the form you just completed as an attachment.

5.        Use this form to print and mail to the club at the address on the form.  PDF FORM CLICK HERE

When we receive the form we will print it and send an e-mail to the sender advising that we have received Report form.   If there is a problem with or if there is anything that is not clear on the form we will ask for verification of the problem in the e-mail we send you.  NOTE: YOUR REPORT IS NOT COMPLETE UNTIL YOU RECEIVE AN E-MAIL FROM US TELLING YOU THAT THE REPORT HAS BEEN RECEIVED.
